Requirements for Preliminary Submissions

  • 300dpi
  • .jpeg format
  • maximum of 10 submissions
  • must live in 100 miles of Atlanta
  • art work must have been created in the last 3 years, in Atlanta
  • files should be named as follows: FirstNameLastName + 3 digit images number (ex. johndoe001.jpg, janedoe007.jpg, or alecbaldwin010.jpg)
  • Upload files online via Web FTP (or send a CD in the mail). DO NOT EMAIL YOUR FILES, use instructions below.


  • List of Submission Titles With Corresponding File Name (please label submissions correctly)
  • Completed Questionnaire
  • Completed Entry Form

How to Submit


  1. Mail a CD with your files correctly named to Williams-England, 982 Memorial Drive, Atlanta, GA 30316.
  2. You may include .jpg files for your submissions,  and .doc, .rtf, .txt, or .pdf files for your forms and artist info.  Please DO NOT use .docx files.
  3. Celebrate.

OR (preffered) ONLINE METHOD

  1. Go to and log in.
  2. User Name: formbook
  3. Password: williamsengland
  4. FTP server:
  5. Select the FORMbookfiles directory.
  6. Make a new subdirectory by selecting the “New Dir” button under the ACTIONS menu.  Name that directory your First and Last name. Select the Green Checkmark. You should see the text “Directory YOUR NAME was successfully created.”
  7. Select the Blue Arrow to go back to the “FORMbookfiles” directory.  Select your newly created directory from the NAME column.
  8. Press the UPLOAD button in the ACTIONS menu and browse the files necessary on your computer.  Only place files in your directory.  You may upload .jpg files for your submissions,  and .doc, .rtf, .txt, or .pdf files for your forms and artist info.  Please DO NOT use .docx files. Select the Green Checkmark and finish up.
  9. You will get a confirmation email from “” within 24 hours and your files will be taken off the server.  If you do not receive your confirmation email within 24 hours, please email “” for help.  Remember to add “” to your email’s address book so our email doesn’t get marked as spam.
  10. Celebrate.

After You Submit

Submissions close Monday, September 14th.  If your work is chosen for inclusion in FORM Artistic Independence 2009, you will be notified via email within two weeks of the closing date.  There will be detailed instructions on what to do then.  Once work is chosen, artists reserve right to submit any or all works chosen.  Credits for all work will be given to artist pertaining to their work.
